Mohammed Tae Kwon Do LLC – Participant Waiver & Release of Liability

By registering for and participating in Tae Kwon Do classes, programs, or events offered by Mohammed Tae Kwon Do LLC (“MTKD”), the location the classes are held in, you (“the Participant”) agree to the following terms:

 

1. Assumption of Risk

I acknowledge that participation in martial arts involves inherent risks, including but not limited to physical contact, strenuous activity, and the risk of injury. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including serious injury or death, that may be sustained as a result of participating in these activities.

 

2. Release of Liability

I hereby release, waive, discharge, and hold harmless Mohammed Tae Kwon Do LLC, its instructors, employees, agents, volunteers, and representatives, as well as any location the classes are held, its staff, board, and representatives, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or harm that may be sustained during participation in Tae Kwon Do classes or related activities.

3. Medical Fitness to Participate

I certify that I am physically fit and have no medical condition that would prevent safe participation. If I have any doubt about my health, I will consult a physician before participating.

 

4. Emergency Medical Authorization

In the event of a medical emergency, I authorize the instructor(s) to obtain medical treatment deemed necessary for myself or my child, at my own expense.

 

5. Indemnification

I agree to indemnify and hold harmless Mohammed Tae Kwon Do LLC against any loss, liability, damage, or costs they may incur due to my participation or my child’s participation in the program.

6. Photography & Media Release

I grant permission for photographs or videos taken during classes to be used for promotional purposes by Mohammed Tae Kwon Do LLC.

7. Acknowledgment

By registering and checking the box on the registration page, I confirm that I have read, understood, and voluntarily agree to these terms. I understand this is a legally binding release of liability.
